In the past two decades a number of studies have shown that abnormalities in the function and structure of coronary microcirculation can be detected in several cardiovascular diseases. On the basis of the clinical setting in which it occurs, coronary microvascular dysfunction (CMD) can be classified into four types: CMD in the absence of any other cardiac disease; CMD in myocardial diseases; CMD in obstructive epicardial coronary artery disease; and iatrogenic CMD. In some instances CMD represents an epiphenomenon, whereas in others it represents an important marker of risk or may contribute to the pathogenesis of myocardial ischemia, thus becoming a possible therapeutic target. This book provides an update on coronary physiology and a systematic assessment of microvascular abnormalities in cardiovascular diseases, in the hope that it will assist clinicians in prevention, detection and management of CMD in their everyday activity.

About the Author

After his degree in medicine, Professor Crea specialized in cardiovascular medicine and in respiratory diseases at the prestigious Scuola Superiore di Studi Universitari of Pisa. In 1984 he was Research Fellow at the Cardiology Division of the University of Florida, and from 1985 to 1992 he worked at the Cardiovascular Research Unit of Hammersmith Hospital di Londra, directed by Professor Attilio Maseri. Since 2000, he is director of the Cardiology Department and of the School of Intensive and Subintensvie Cardiology at the Catholic University of Rome. He is Fellow of the American College of Cardiology and of the European Society of Cardiology, and Chairman of the ESC Working Group on “Pathophysiology and Microcirculation”. He is Associate Editor of Heart, Editorial Board member of Circulation, Clinical Cardiology and Revista Espagnola de Cardiologia. In 1992 he received from Professor Rita Levi Montalcini the Newburgh Award for the “the exceptional scientific contribution in the field of cardiovascular research”. His main research interests are: 1) mechanisms of ischemic cardiac pain; 2) mechanisms of myocardial transient ischemia; 3) acute coronary syndrome pathogenesis delle sindromi coronariche acute. He is author of more than 200 pubblications on indexed journals.
